PAYING YOUR MEMBERSHIP DUES  

Starting October 1, 2009, dues to MABE will be due monthly instead of quarterly at a rate of $60 each month.    

You have three ways to pay the monthly MABE dues.  Every member will get a monthly bill, sent by email, regardless of the option selected.

OPTION 1 – Direct Automatic Debit
Under this option, your bank account will automatically have a debit of $60 each month.   If you would like to take advantage of this option, please do the following:

1)      Complete the Raymond James ACH Transaction Request form.  Under the section FINANCIAL INSTITUTION INFORMATION, fill in the information for your checking or savings account.

2)      Attach a voided check. 

3)   Sign the form under the section CLIENT ACKNOWLEDGMENTS AND SIGNATURES.  Sign and date the form in the box that is second from the bottom on the right side, marked with an “X” and titled “Financial Institution Account Holder Signature.”  If the account is a joint account, the second signer on the account must also sign the form on the line directly below.

4)   Mail all of the following to MABE at 8490 S. Power Rd., #105 PMB 213, Gilbert, AZ 95297:

·         The completed and signed form

·         Your voided check

OPTION 2 – On-Line Banking

For those members who have online banking, you may prefer to set up recurring monthly payments of $60 from your bank account.  Your online banking account needs to be set up with MABE as a “Pay to Account.” 

The monthly payments made through your online banking account are payable to MABE and sent to 8490 S. Power Rd., #105 PMB 213, Gilbert, AZ 95297.                

OPTION 3 - Check

Pay your monthly MABE dues of $60 the old-fashioned way.  Each month, issue a check to MABE and send to 8490 S. Power Rd., #105 PMB 213, Gilbert, AZ 95297.
